Eligibility: NUS Students and Staff only.
As a participating team of NUS Community Sports Carnival 2018 (hereinafter referred to as the "Event"), we understand and will abide by these terms and conditions and the Rules and Regulations found at [provide website address] established by NUS Community Sports Carnival 2018 Organizing Committee (hereinafter referred to as the “Event Organizer”). The Event Organizer is from NUS Office of Student Affairs.
We are aware that the following activities (hereinafter referred to as the "Activities") are available for our team’s participation during the Event:

DBS Battle Bay
We acknowledge that the Activities may subject to change without prior notification to us
We acknowledge that we are solely responsible for our team’s possessions and athletic equipment during the Event and its related activities.
We acknowledge that the contents of the Goodie Bag may subjected to change by the Event Organizer without prior notification to us.
We consent to photographs and video-recordings to be taken of anyone of us during the Event for the purpose of use in publications and social media posting of NUS. We acknowledge and agree that the copyright and all other intellectual property rights in and to the photographs, video-recordings or any other records taken of anyone of us for the Event (the “Material”) shall vest solely and absolutely in NUS.
We acknowledge that the in exchange of any prizes or gifts upon completion of the stipulated number of activities, body ink stamp shall be inked on our skin, and it is our duty to inform the Event Organizer at the respective Activities booth that anyone of us has medical condition that the ink could cause adverse effect in anyone of us.
We agree that in the event of an event cancellation, or any Activities cancellation, due to conditions beyond the control of the Event Organizer, including but not limited to inclement weather, such as thunderstorm, rain, strong winds or haze, or acts of terrorism or god, our registration fee will not be refunded to us.
The Event Organizer reserves the right to disqualify anyone of us if we were caught cheating or dishonest during the Activities. No refund of registration fees will be made to us in the event of disqualification.
The Event Organizer reserves the right to modify the Conditions and Rules and Regulations at its sole discretion and without prior notification of the participants.
We are aware that the ticket sold to the team is neither transferable nor refundable.
Any changes to the participants in the team are subjected to approval by the Organising Committee. Please submit your request to firstname.lastname@example.org at least 3 working days before the event. Late submission with appropriate reasons will only be considered at the discretion of the Organising Committee.
We acknowledged that we are required to submit the necessary forms (Indemnity Form/Health Declaration Form/ Swimming Declaration Form) for all three of us during the actual day. Failure to do so may result in our team disqualification, and no refund of registration fees will be made to us.
